If you are in a nursing facility or rehabilitation hospital, your facility staff can refer you to our care team for a new patient visit. If you are in assisted living or independent living, you will need to complete our New Patient Packet and return it to us along with copies of your insurance cards. Visit frequency is individualized for each patient and based on medical necessity (as defined by Medicare). But as a generalization, the following standards have historically held true and resulted in care that is both timely and medically appropriate.

Yes. We strive to visit patients with a frequency that promotes timely proactive medical management, while at the same time avoiding unnecessary encounters and medical expense. If patients feel they are being seen too often, or not often enough, we are always happy to discuss.

IF YOUR PLAN IS NOT LISTED ABOVE, we would still like to see you. We are typically able to negotiate an arrangement with insurance carriers new to our practice. And we accept only the amount allowed by insurance for out-of-network claims, without balance billing our patients. As required by law, we do bill patients for co-pays, co-insurance, and any other amounts mandated by their health plan. However, Meridian patients are never denied care or referred to a collection agency if they are unable to meet their self-pay obligation.
